Provide direct services for financial and alternate health resource. The PBC shall insure the services readily available to the patient, the patient’s family and other persons significant to the patient. The PBC program operations are limited, but specifically incorporate the following responsibilities and activities:
- Social Security application assistance
- SSI assistance
- Information and referral services
- Involve in community prevention activities
- Active interaction with the MSU
- Extensive Case-Management activities
- VA referrals and assistance
- Medicaid, Food Stamps and LIHEAP applications assistance
